    Description

    Job Summary:
    Works with Hospice Management and field team to ensure integrity of program. Under direct and indirect supervision, analyses referral requests/authorization for Hospice/HBPC patients in Longview and Salem. Provides technical and administrative coordination for Hospice and Palliative Care Program Director, Team Managers, Volunteer Manager and Bereavement Coordinators. Responsible for data coordination of hospice information across care settings including billing.


    Essential Responsibilities:

    • Plans, develops and maintains data base and reports for hospice and palliative care programs. Identifies opportunities to redesign existing processes to improve quality and efficiency.

    Responsible for maintenance and coordination of program/patient information:
    i.e .census, demographics, billing source, bereavement information, inpatient stays; as defined by CMS. Responsible for monitoring, auditing, reconciling of daily, monthly and yearly program reports.


    • Ensures timely and regulatory compliant transmission of information to CMS, PFS and other KP departments. Assists Team Managers to ensures CMS compliant Hospice admission process.
    • Collaborates with Hospice/HBPC Director to assist with regional over sight of patients receiving care from outside Hospice/Palliative vendors. Collaborates with Claims Administration and Hospice Management on CMS billing, co-pays and patient care authorizations to ensure timely payment and problem resolution. Communicates benefit information to Hospice and Palliative care contract providers. Manages the authorization process for Hospice/HBPC services in Longview and Salem. Secures spot contracts as needed.
    • Assist the Hospice Bereavement Program to maintain CMS regulatory requirements including the oversight and maintenance of Bereavement data base. Coordinates hundreds of monthly condolence and bereavement mailings, Tracks and processes changes for the regions support and grief groups. Assists with coordination of yearly Light Up A Life and other scheduled memorial events.
    • Oversee the volunteer clerical Internship Program including organizing and directing of the volunteer clerical interns and hospice clerical volunteers.
    • Under the direction of the Volunteer Manager, is responsible to maintain the volunteer personnel file documentation system, to meet all state, federal and Joint Commission accreditation requirements.
    • Assists with research and provides support for the Hospice Quality Program.
    • Processes, and records all hospice donations to ensure accurate monthly reconcile as outlined by accounting. Timely acknowledgement letters to contributors and families.
    • Perform other duties as requested.

    Basic Qualifications:
    Experience


    • Minimum two (2) years prior experience working with Hospice, Palliative Care or equivalent experience in another healthcare organization.
    • Minimum two (2) years experience with progressive responsibilities in project work.
    • Minimum two (2) years PC experience to include 1 year database design and management.
    Education


    • Associate degree in Computer Science, Business Statistics, Health Care, OR two (2) years of experience in a directly related field.
    • High School Diploma or General Education Development (GED) required.

    Additional Requirements:

    • Working knowledge/comfort in standard medical terminology.
    • Experience working independently in a multitask environment.
    Working knowledge of medical terminology ,ICD-9 and ICD10 coding.


    • Thorough knowledge of microcomputer applications, database design and management, word processing software and spreadsheets.
    • Working knowledge of medical office functions.
    • Working knowledge of budgetary functions and procedures.
    • Demonstrates customer-focused service skills.
    • Type minimum 60 WPM with high level accuracy.
    • Able to be self-reliant and assume initiative in organizing, controlling and prioritizing multiple activities in a busy office environment.
    • Excellent oral and written communication skills.
    • Ability to create and maintain database systems to support data tracking and report generation.
    • Ability to manage multiple assignments and priorities.
    • Ability to think critically, analyze accurately, problem-solve and make appropriate decisions.
    • Ability to apply specialized clerical knowledge to a wide variety of situations.
    • Excellent organizational skills.
    • Excellent interpersonal communication skills.

    Preferred Qualifications:

    • Working knowledge of OA.
    • Experience with database programming in two or more database systems (including Microsoft Access).
    • Experience with maintenance of personnel files.
    • Minimum three (3) years experience in hospice/palliative care.
    • Experience in training and supervising volunteer personnel.
    • Knowledge of Medicare Hospice rules and regulations.
    • Thorough knowledge of Kaiser Permanente organization and structure, medical terminology.
    • Working knowledge of hospice budgetary functions and procedures.
    • Ability to assemble required data and develop high-level complex reports.
