    TITLE: On-Track Coordinator

    QUALIFICATIONS:
    1. State of Connecticut certification in School Social Worker (071).
    2. Master's degree required.
    3. Five years of successful teaching experience preferred, but not required.
    4. Strong interpersonal, communication, and organizational skills and ability to work collaboratively with administrators, staff, community partners, students, and families in the provision of high-quality services and resources.
    5. Ability to multitask, prioritize, problem solve, organize work, meet deadlines, follow directions and take initiative.
    6. Maintain a strong belief in the East Hartford Public School district's core values and IB programmes at CIBA.
    7. Such alternatives to the above qualifications as the Board of Education may find appropriate and acceptable.

    REPORTS TO: Department Head and Principal

    JOB GOAL: The On-Track Coordinator's role is to fully prepare CIBA students to meet the demands of college/university through early intervention. More specifically, this position focuses on working closely with a caseload of identified ninth graders who are at-risk of not graduating on time based on three indicators: grades, behavior, and attendance. By providing a strong foundation during grade 9 and supporting them in their transition from middle school to high school, students will be able to have a successful start to their high school career. The On-Track Coordinator will also work with identified students from other grade levels on Social Emotional Learning ("SEL") needs.

    PERFORMANCE RESPONSIBILITIES:
    1. Identify grade 9 students in need of support (based on grades, behavior, and attendance) through staff recommendation, middle school records, and PowerSchool data.
    2. Conduct individual student needs assessments in order to identify trends.
    3. Analyze a variety of student data to create targeted interventions that meet identified student needs.
    4. Implement interventions with fidelity to provide support to students in small groups and 1:1 based on their needs and achieve identified goals and objectives.
    5. Facilitate small groups around SEL needs.
    6. Engage students on caseload on a regular basis, including daily check-ins (at the start of the school day), during study halls and/or lunch, and weekly meetings (review attendance, academics and behavior data).
    7. Oversee Scholar Hour on a quarterly basis.
    8. Create and maintain a log of supports/interventions including frequency (date/time) and results to track progress.
    9. Assess student needs/progress on a continual basis and adjust the intervention/strategies for each student on caseload (as necessary).
    10. Facilitate "on-track" data meetings.
    11. Communicate with students, family members, school personnel and members of the community.
    12. Create a positive, achievement-oriented and structured learning environment that excites and engages students.
    13. Motivate students to realize high academic and behavioral standards through building relationships with students.
    14. Help students develop positive self-concepts through counseling and mentorship based on academic and/or behavioral goals.
    15. Facilitate the development of student independence and confidence.
    16. Promote a positive, safe, and inclusive learning environment.
    17. Build community by investing families in the child's academic success.
    18. Establish and expand community partnerships and family/school connections.
    19. Facilitate the cooperative involvement of parents and community in the educational process through regular communication, meetings, and home visits.
    20. Participate in professional activities such as workshops, conferences, parent/teacher meetings and activities of other pertinent professional organizations.
    21. Attend all professional development, team planning and data analysis meetings.
    22. Create and present professional development (focused on SEL) as requested by Principal or supervisor.
    23. Assume such other functions as may be determined.
      TERMS OF EMPLOYMENT:
      Salary and work year to be established by the collective bargaining agreement between the Board of Education and EHEA.

      Performance of this job will be evaluated in accordance with provisions of the Board's policy on Evaluation of Certified Personnel.
