- Collaborate closely with individuals and their families to identify goals and access support services that promote their well-being.
- Function as a key member of a care team, liaising with healthcare providers, insurance companies, natural supports, mental health professionals, and other community service providers.
- Ensure seamless coordination and communication among all support sources and services for each individual.
- Coordinate care needs with participants, their natural supports, other service providers, and medical experts.
- Organize regular team meetings to maintain open communication.
- Document all interactions and follow-up activities accurately.
- Facilitate referrals for necessary services.
- Stay up to date with Medicaid and other benefit programs.
- Ensure compliance with program regulations.
- Bachelor's degree in a Health and Human Service field.
- 3 years of Care Management experience with individuals with disabilities, mental illness, or seniors (with a Bachelor's degree); or 1 year of relevant experience (with a Master's degree).
- Proficiency in community resources and service linkages.
- Valid New York State Driver's License.
- Strong communication and collaboration skills.
- Ability to work effectively in a team setting.
- Familiarity with common mental health and substance use disorders.
- Proficiency in computer skills.

Role Overview

Physical Requirements
The candidate must be able to occasionally lift loads of 30 pounds, walk, stand, sit, talk, and hear. Specific vision abilities are also required.
Working Conditions
The job is primarily indoors in an office setting, with exposure to drafts and temperature changes near the building entrance.
