- Provide clinical and administrative support to the MDS Coordinator
- Regulate and maintain the MDS/PPS program and completion schedule
- Monitors MDS and care plan documentation for all residents. Ensures documentation is present in the medical record to support MDS coding
- Develop and coordinate appropriate nursing care plans for all residents
- Evaluate effectiveness of nursing care plans, and update MDS/RAI process within completion guidelines
- Works in collaboration with the Interdisciplinary Team to assess the needs of the resident
- Active and unencumbered VA License as an RN
- Prior MDS and Case Mix experience is preferred
- Proficient user of computer - Microsoft Windows, Word, and Excel
- Ability to communicate effectively with residents and their family members, and at all levels of the organization
- Ability to organize and prioritize
- Ability to maintain confidentiality
- Ability to be accurate, concise and detail oriented
- Ability to work cooperatively as a member of a team
