About The SBY Therapeutic Group Home


The Sasha Bruce Therapeutic Group Home is one of Sasha Bruce's newest programs serving children and youth ages 13-21 from foster care who present with an Axis 1 diagnosis.

The program has a high level of structure, supervision, behavior management/modification, and clinical intervention due to the nature of the emotional and behavioral conditions.


Position Summary
The Linkage to Care Coordinator will report to the Program Manager of the Sasha Bruce Therapeutic Group Home. They will provide care coordination, social service navigation and support to clients.


Qualifications

  • License/Certification: Bachelor's Degree in Social Work, Counseling, psychology, or another relevant Human Services field preferred OR 3 experience in the human service field preferably working with adolescents and/or young adults required;
  • A minimum of 2 years of extensive experience with at risk youth;
  • Competency in crisis intervention and deescalation;
  • Strong communication and organizational skills required; and,

Duties and Responsibilities

  • Assess eligibility for communitybased services and refer clients to appropriate services as based on Individualized Treatment Plans and Behavior Modification and Management Plans.
  • Collaborate with families, schools, law enforcement and emergency services as well as other agencies and community partners to coordinate and/or facilitate needed services.
  • Provide support in the creation of Individualized Treatment and Behavior Modification and Management Plans for clients in programming.
  • Advocates for and actively assists clients in obtaining services; serve as liaison between client and service providers when appropriate.
  • Specifically provide substance abuse supports and refer youth to substance abuse counseling when appropriate; provide referrals to grief, loss and separation counseling to support clients.
  • Maintain familiarity with local services (both within and outside the Agency) that might serve as resource.
  • Complete service documentation in client case records within 2448 hours of the service; maintain accurate, complete, and confidential case files, written documentation, reports to track client progress and outcomes and all other necessary clinical documentation.
  • Participate in regular biweekly team consultation meetings with the Clinician, Behavior Modification Technician, case manager and other staff working with shared clients.
  • Participate in staff meetings, trainings, conferences, regular supervision, and community outreach events as required.
  • Other duties as assigned; and,
