- Establish trusting relationships through mobile/street outreach to assist homeless individuals in overcoming barriers to housing, services, and healthcare.
- Provide transportation and guidance to homeless individuals for enrollment in the NAMI SD/IC Clubhouse.
- Document member progress, review goals, and provide necessary support.
- Conduct assessments and connect members to appropriate interventions and services.
- Assist members with benefits, documentation, housing referrals, and health-related support.
- Collaborate with law enforcement and service providers for a smooth transition to interim housing.
- Maintain mobile navigation vehicle and provide case management services within the Clubhouse and the community.
- Record and update member profiles and goal sheets regularly.
- Additional duties as assigned.
- Bachelor's degree in a relevant field and experience working with homeless populations or vulnerable groups.
- Fluency in English and one additional language from a specified list.
- Previous experience in homeless outreach.
- Basic computer skills and ability to maintain confidentiality.
- Must have a satisfactory driving record and strong communication skills.
- Physical ability to meet job requirements.
- Prior experience with mental health or housing insecurity preferred.

Job Title: Bilingual Homeless Outreach Worker
Description: The Bilingual Homeless Outreach Worker will be responsible for delivering mobile, street, and field-based outreach services in the Central Region of San Diego County, in collaboration with the NAMI San Diego & Imperial Counties Clubhouse and other service providers.
Duties and Responsibilities:
Reporting: The position reports to the Program Manager.
Requirements:
