Summary
The Outreach Worker Medical Assistant will be responsible for the provision of street and clinic outreach services.

This includes short-term crisis intervention, case management, and counseling for clients experiencing homelessness, substance use disorder (SUD), and serious mental illness (SMI) as well as other barriers to housing stabilization.

This position also supports the work of licensed clinicians both in clinic and in the community by obtaining vital signs, administering medications and injections, recording information in electronic medical records, preparing, and handling medical instruments and supplies, collecting and preparing specimens of bodily fluids and tissues for laboratory testing.

The outreach worker will collaborate with the street health team and contribute to street health efforts.


Essential Functions

  • Locates and connects homeless individuals/families on the streets in order to guide them through the coordinated entry system and/or connect with the Street Health Program and Village Health Center services.
  • Works with internal and external parties to provide clients with community resources, and aligns clients with resources consistent with insurance benefits to help improve patient health status.
  • Collects and enters patient information, status, and statistics for reporting purposes in the electronic health record and various reporting databases to improve the efficiency of services.
  • Performs direct patient outreach in the community by searching the lunch line, walking the neighborhood, checking tents and cars, and working with the patient to return to the Health Center for follow up, as needed, to promote effective coordination of care.
  • Recording information in electronic medical records, collecting and documenting patients' medical information, including medical history to ensure effective coordination of care.
  • Multitask, communication with patients/deescalation & assessment skills, provide customer service, and traumainformed care to ensure patient safety and satisfaction.
  • Preparing and administering medications and injections under the direction of licensed clinicians.
  • Prepping exam rooms for clinical procedures and assisting clinicians with patient care, collecting and preparing specimens of bodily fluids and tissues for laboratory testing, prepping exam rooms for clinical procedures and assisting clinicians with patient care, preparing and handling medical instruments and supplies to improve patient outcomes.
  • Scheduling patient appointments and maintaining communication to confirm future appointments, verifies & cancels prescriptions at the pharmacy, and scans and faxes documents.
  • Ontime completion of assigned training and policies.
  • Performs other duties as assigned.

Qualifications

  • High School Diploma or GED
  • Medical Assistant Certificate
  • At least 1 year of outreach and/or case management experience working with underserved populations.
  • Basic user of MS office
  • Participate in annual Tuberculosis screening and/or other screenings when necessary.
  • Reasonable accommodations may be granted where appropriate.
