Description
Job DescriptionUnder the general direction of the Manager, Clinic RN, Care Manager RN facilitates communication between caregivers and providers in the advocacy of the patient. Acts as a liaison between staff and interdisciplinary teams to support coordination of patient care. Tracks the ordering and completion of studies and steps necessary prior starting treatment (imaging, pathology, staging, etc. based on treatment type) to ensure timely authorization and treatment start. With direction from the providers, coordinates and facilitates admission for in-hospital treatment and seamless transition back to outpatient treatment.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
