- Identify needs of COVID-19 patients and connect patients with resources such as local food banks, mental health services, and other programs to help patients and their families during quarantine and isolation.
- Utilizes public health education resources to inform patients and communities about safety measures, minimizing spread, isolation and quarantine guidance, monitoring their symptoms, and seeking medical care.
- Works closely with the epidemiology and population health teams to strategically target efforts towards high-risk communities.

f. Builds and maintains relationships with community champions, organizations, businesses.
