As a part of our team, you will coordinate, facilitate, and execute interdisciplinary care plans by assessing and evaluating patient conditions and care effectiveness under the guidance of the attending physician. This involves traveling to patient homes, Community-Based Residential Facilities (CBRF), assisted living facilities, hospitals, or other locations to meet patients and their families/caregivers, assess care needs, and ensure compliance with home care and hospice service regulations.
Key Responsibilities:
- Assess and coordinate patient discharge planning needs
- Perform point-of-care testing following protocols
- Collaborate and communicate with physicians on care plans and patient status updates
- Ensure accurate documentation and coordination of care plans with multi-disciplinary teams and external agencies
- Conduct comprehensive medical assessments and data entry
- Deliver skilled nursing care as per the care plan, including vital sign monitoring, wound care, IV care, ostomy care, blood draws, injections, and overall patient condition assessments
- Document visit notes, treatments, and care provided
- Work with patients, families, and healthcare teams to develop tailored care plans based on individual needs
Requirements:
- Graduate of an accredited school of nursing or equivalent education for licensing
- No prior experience necessary
- Proficiency in keyboard use and data entry
- Basic Life Support HealthCare Provider (BLS HCP) certification - American Heart Association (AHA)
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Registered Nurse (RN) - Missouri Division of Professional Registration
- Registered Nurse (RN) - Oklahoma Board of Nursing (OBN)
