Position Description:


About Harris County Community Services Department:

  • This office is focused on addressing the housing, infrastructure and public service needs of the low to moderate-income community in Harris County. This is accomplished through a process of securing and managing grants from the U.S. Office of Housing and Urban Development (HUD) and partnerships with other government and non-profit agencies, resulting in a variety of programs, projects, and services.

Case Manager Overview:


  • The Case Manager assists clients with a range of activities including, but not limited to, housing stability needs, financial assistance, location of shelter or housing, job preparedness, etc. The Case Manager completes interviews to identify client barriers and issues, such as medical, emotional, and social factors that contribute to the client's situation, through the completion of comprehensive assessments and housing plans. The Case Manager will often assist clients who have issues including, but not limited to abuse, neglect, domestic violence, mental health, alcohol/substance use, HIV, trauma, and/or involvement with child protective services. Extensive documentation and data entry is required.

Duties & Responsibilities:


  • Communicating with agencies including Housing Authority and Houston Coalition for the Homeless.
  • In field work, phone and or virtual meetings, including but not limited to, monthly home visits, phone and/or virtual visits with clients, land lord contact, contact with Housing Authority, social service agencies, check delivery, housing search, meet with clients at encampments, meetings, trainings, attend court, and shelter visits etc.
Referring and linking clients to resources/services in the community Disaster related assignments and or other duties as assigned Perform other duties as assigned. Requirements:
Education and Experience:


  • High school and (5) years of experience working with vulnerable populations
    OR
  • Associates Degree and (3) years of experience working with vulnerable populations
    Knowledge, Skills, and Abilities:
  • Knowledge of the NASW Code of Ethics as a guide to decision making and promoting ethics and values of the social work profession.-
  • Knowledge of the Fair Housing Act-
  • Ability to engage with clients and exhibit nonjudgmental attitude and compassion.-
  • Ability to use critical thinking skills to assess situations and assist clients.-
  • Ability to work with vulnerable populations, such as homeless persons

Preferences:


  • 1 year of experience working in the Child Protective Services Sector or with foster children 1 year of experience working in any HUDfunded grant program Experience with field work, such as home visits with clients. Experience working in the HMIS system Trained in Trauma informed Care Trained in Motivational Interviewing
    General Information:
    Position Type and Typical Hours of Work:


  • Full-time; Monday

  • Friday; 8:00 AM
  • 5:00 AM; Weekends (if necessary)
    Work Environment and Physical Demands:


  • Cubical workspace

  • Field work & Home Visits
  • Use of County vehicle
  • Must follow Harris County and CDC guidelines for COVID19
  • Some sedentary work, driving to complete field work including driving a Harris County Car and or your own vehicle.
  • Field work can include and is not limited to: home visits, agency visits, verifying homeless status in outside elements, up and down stairs at apartment complexes, driving a county pool car, wearing protective personal equipment when needed.
    Employment may be contingent on passing a drug screen and meeting other standards.
