    Hospice Intake Coordinator

    About the job:
    The Hospice Intake Coordinator collaborates with hospital discharge planners/social workers, physicians, billing specialists, patients, families, and others to receive information which facilitates admission of Hospice patients; documents appropriate demographic and clinical information for the processing of incoming referrals; coordinates communication process between


    referral sources, third party payors, and professional/clerical staff; and coordinates referral process from initiation to completion including all follow-up necessary to accomplish expected outcome.


    What we are looking for:

    • High School diploma/GED
    • Experience: Minimum of one year recent Hospice experience or related setting such as home health, hospital, ambulatory care center, physician office in which assessment and critical decision-making skills are required
    • Effective Communication Skills
    • Demonstrated ability regarding critical thinking skills, telephone and direct assessment of patient, responding to emergency referrals, establishing priorities re: scheduling admissions of Hospice patients in all settings-home, hospital, nursing home, and assisted living center.
    • Must be able to work a flexible schedule; may be required to work evenings, weekends, holidays, and on-call.
    • Effective Interpersonal skills, demonstrates tact, judgment and diplomatic business-like manner in dealing with physicians, other healthcare professionals and coworkers.
    • Working knowledge of personal computer and windows-based software applications including accessing and sorting appropriate data repository information related to Hospice referral.
    • Knowledge of reimbursement sources for health care (Medicare, Medicaid, other third party payment sources).
    • Knowledge of Hospice settings of careunderstands the durable medical equipment necessary to meet specific needs of population served.
