- Facilitate the referral/ intake process, and maintain admission data input
- Assist as needed with Daily census & Population data, schedule admissions, and other admissions reports.
- Conduct reminder calls prior to admissions
- Maintain admissions checklist and documentation on checklist to ensure adequate documentation
- Follow up and maintain communication with referral sources upon admission and post admission
- Document demographic information of clients in Welligent (i.e.: drug history, demographic, etc.)
- Perform other duties as assigned.
- Exposure to clients infected with Hepatitis B, HIV, TB, or other Infectious Diseases
- Business-related travel may be required

SUMMARY
The Access Assistant shall oversee the efforts of the Exeter Programsintake function, ensuring an efficient and professional process of screeningand placing our client referrals with a concentration on the Exeter Programs. The AccessAssistantshall also be responsible for the screening, assessment, and referral ofpotential admissions to an appropriate level of care.
ESSENTIALDUTIES AND RESPONSIBILITIES FOR ACCESS COORDINATOR POSITION
·Demonstrate empathy, compassion, andrespect for clients in all interactions.
·Maintain objectivity towards allclients and make decisions based on accurate information.
·Answer phones and transfer to appropriatetreatment provides as needed.
·Maintain documentation related to MAP and/orAccess Team tools (ie: Communication Log, call tracking, etc)
·Demonstrate appropriate boundariesbetween self and clients.
·Serve as a rolemodel through positive, appropriate conduct and participation inspecial agency events.
·Consult with Program Clinical Teamand/or Nursing to review potential referrals as needed
·Project a good image to the public.
·Complete E-learning requirements in atimely fashion.
·Dress in a professional manner.
·Work cooperatively with others andsupervisors.
·Demonstrate patience and considerationin dealing with others.
·Demonstrate thoughtful decision-making.

QUALIFICATIONS

EDUCATION/EXPERIENCE: Bacheloror Associate Degree in related field from an accredited institution orequivalent combination of education and experience.
