Social Work Care Manager you will be a community advocate and liaison by helping patients and their families gain access to the essential services that can transform their life.

Social Work Care Manager Responsibilities:

Conduct comprehensive assessments of patients' behavioral health needs
Develop individualized care plans based on assessment findings
Collaborate with interdisciplinary teams to coordinate and implement patient care
Monitor and evaluate patient progress, adjusting care plans as needed
Provide education and support to patients and their families regarding behavioral health conditions and treatment options
Advocate for patients' needs and ensure access to appropriate resources and services
Document all patient interactions and interventions accurately and timely

Social Work Care Manager Skills:

Strong knowledge of behavioral health conditions, treatments, and interventions
Ability to assess and manage crisis situations effectively
Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals
Proficient in using electronic medical record systems for documentation purposes
Ability to work independently as well as collaboratively
Strong organizational skills to prioritize tasks and manage time effectively
