As a
Social Work Care Manager you will be a community advocate and liaison by helping patients and their families gain access to the essential services that can transform their life.
Social Work Care Manager Responsibilities:
- Conduct comprehensive assessments of patients' behavioral health needs
- Develop individualized care plans based on assessment findings
- Collaborate with interdisciplinary teams to coordinate and implement patient care
- Monitor and evaluate patient progress, adjusting care plans as needed
- Provide education and support to patients and their families regarding behavioral health conditions and treatment options
- Advocate for patients' needs and ensure access to appropriate resources and services
- Document all patient interactions and interventions accurately and timely
Social Work Care Manager Skills:
- Strong knowledge of behavioral health conditions, treatments, and interventions
- Ability to assess and manage crisis situations effectively
- Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals
- Proficient in using electronic medical record systems for documentation purposes
- Ability to work independently as well as collaboratively
- Strong organizational skills to prioritize tasks and manage time effectively
