    NYC Health + Hospitals/Carter (EAST HARLEM) is a leading, comprehensive specialty care hospital and nursing facility dedicated to providing quality medical, sub-acute, rehabilitative and long-term specialty services. NYC Health + Hospitals/Coler (ROOSEVELT ISLAND) is an 815 bed, leading nursing facility dedicated to providing quality medical, sub-acute, rehabilitative and long-term specialty services. We are committed to serving New York City, providing services to individuals requiring geriatric, memory care and rehabilitation services. NYC Health + Hospitals/Gouverneur (MANHATTAN) provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly and infirmed in its 295-bed Skilled Nursing Facility (SNF). NYC Health + Hospitals/McKinney (BROOKLYN) provides high quality short-term rehabilitation and long-term skilled nursing care in a comfortable person-centered setting. Each resident has a qualified and caring team of physicians, nurses, dieticians, and therapists dedicated to his/her physical, mental, and emotional well- services include physical therapy, occupational therapy, speech and audiology, and vocational rehabilitation.
    Job Description:
    • Assists RN in admission procedure and participates in the planning, implementing and evaluation nursing care.
    • Sets priorities and delivers directs care to meet the needs of the assigned residents.
    • Provide medications, treatments and perform nursing procedures safely. Document per policy and procedure in resident's medical record.
    • Participate in discharge regime and provide necessary teaching to resident and family on a regular basis.
    • Demonstrate safety awareness and follow protocol for fire, disaster, infection control and other environment of care practices.
    • Maintain competency and proficiency in nursing resident care standards. Attend all mandatory classes, and participate in the continuing education program and other appropriate learning experiences.
    • Participate in performance improvement activities and other committees as assigned.
    • Performs other related duties.
    • Supervise Nursing Aides as directed and assigned.

    Minimum Qualifications:
    • Currently licensed or otherwise duly authorized to practice as a Licensed Practical Nurse in the State of New York.
