MAJOR FUNCTIONS:The
Licensed Practical Nurse (LPN) is responsible for oversight of the medication administration process. She/he supervises the scheduling of medical/psychiatric appointments for all residents. The Licensed Practical Nurse participates as an active member of the Interdisciplinary Team, providing nursing and public health care services to program residents and ensuring that all medical appointments are kept as scheduled.


SPECIFIC DUTIES & RESPONSIBILITIES:


  • Administer medication and document appropriately in Medication Administration Record (MAR).
  • Ensure the accuracy of the MAR at all times and enters all appropriate notations in the MAR regarding the administration or lack thereof for each client and each client's medication.
  • Advise Providers in advance when there are no refill orders left on maintenance medications.
  • Fax, phone, electronically deliver, or by any other practical means, advise pharmacy of medications to be filled.
  • Continuously reviews medications in medication cart to remove expired or redundant medications.
  • Ensure the E-Boxes are indate and complete at all times.
  • Communicate regularly with clinical staff regarding residents who are noncompliant with medication schedule.
  • Educate, train and supervise residents participating in the Medication Independence Training Program.
  • Instruct and educate patients about pharmacological/medical effects of any medications they take.
  • Triage medical problems and emergencies; document appropriately.
  • Monitor and document acute and chronic conditions of residents (i.e., blood pressure, glucose level, etc.).
  • Review and prepare medical consultation forms (Feedback Forms) for residents with outside medical appointments.
  • Assure that charts are in correct order; notes are complete, legible, signed and dated.
  • Check emergency equipment on a monthly basis, and document appropriately.
  • Maintain inventory and records of all medications and medical supplies in clinic.
  • Establish and maintain professional relationship with nonmedical staff to ensure execution of developed treatment plans of residents.
  • Provide monthly health education seminars to residents according to supportive services schedule.
  • Attend regularly scheduled staff meetings and case conferences.
  • Attend all required inservice training seminars.
  • Participate in quality improvement activities.
  • Other duties as required.




REQUIREMENTS:

EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES:


  • Minimum of (1) year experience working in a social service setting required.
  • Current New York State LPN License required.
  • Current CPR & First Aid Certificate required.
  • Proficiency with computer operation (Microsoft Word, Excel, AWARDS, and Outlook programs) required.
