    DESCRIPTION

    As a **Case Manager** working for Molina Healthcare Services (HCS), you will play a crucial role in supporting members with Serious Mental Illness (SMI) in remote and field settings. Here are the key responsibilities of the role:
    • Conduct face-to-face assessments at member's homes
    • Participate in interdisciplinary care team meetings to develop personalized care plans
    • Ensure timely assessments and evaluations, including 'trigger assessments' post-hospitalizations
    • Utilize excellent computer skills and attention to detail to manage multiple tasks efficiently
    • Travel to surrounding areas, up to 25%, for member visits

    Job Summary

    SKILLS & ABILITIES
    • Complete clinical assessments within regulated timelines
    • Develop and implement personalized care plans in collaboration with healthcare professionals
    • Conduct necessary visits and monitor care plan effectiveness
    • Promote the integration of services for holistic patient care
    • Facilitate interdisciplinary care team meetings

    JOB QUALIFICATIONS

    EDUCATION:

    Completion of an accredited LVN/LPN program or Bachelor's/Master's Degree in relevant fields

    EXPERIENCE:

    1-3 years in case management, medical, or behavioral health settings

    LICENSE & CERTIFICATION:

    Active and unrestricted license as required by the job

    Valid driver's license with good driving record

    PREFERRED:

    3-5 years of experience in relevant settings

    Additional certifications such as LCSW, CCM, or LPC/LPCC
