- Maintain caseloads for youth aged 0-18, consisting of 8 to 12 clients.
- Screen and process referrals for admission.
- Collaborate with Mental Health Therapists in creating treatment plans.
- Support foster parents in educating youth on life and independent living skills.
- Coordinate and implement treatment plans for each youth.
- Establish aftercare plans and monitor their progress.
- Complete necessary assessments and documentation.
- Manage and update case files regularly.
- Assist in training and supporting foster parents.
- Participate in agency meetings and events.

- Clear criminal history and tuberculosis test.
- Completion of required certifications within specified timelines.
- Possession of valid driver's license and reliable transportation.
- Health certifications including CPR, First Aid, and Bloodborne Pathogens.
- Proof of COVID-19 vaccination.

Job Title: Therapeutic Case Manager at Foster Care Services
Company: Providing Supportive Solutions
The Therapeutic Case Manager offers a comprehensive range of case management services to young individuals in both Therapeutic and Regular Foster Care settings. Using a trauma-informed approach, the Case Manager coordinates services and case management for all youth under their care referred to the program through specific divisions. The Case Manager conducts regular meetings with the youth, leads group sessions when requested, and collaborates with various stakeholders such as Social Workers and legal advocates to develop personalized housing stabilization plans for each youth.
