Participates in the coordination and access to care for members in the Intensive Care Coordination program. Performs patient assessment and delivers appropriate interventions. Evaluates available resources with support of the Health Resource Coordinator. Plans and implements necessary services and monitors outcomes. Provides education, counseling and on demand information to patients. Provides program education and coordination of care needs with the patients primary care provider. Services are performed in person or telephonically and entered into the Population Care Management Application.Description BSN preferred. Current Alabama License as a Registered Nurse required. BCLS certification required. Position requires a minimum of one (2) years of adult nursing experience. Case management experience in the community setting highly desirable.
Position requires:
Acts only within the scope of his her state license.
Must be able to demonstrate the knowledge and skills necessary to provide care primarily to the geriatric population and, to a lesser degree, the pediatric and adult populations.
Must be able to demonstrate knowledge of the principles of growth and development over the life span.Must be able to assess data reflective of a patients status and interpret the appropriate information needed to identify each patients requirements relative to his her age specific needs.
Registered Nurse (RN) License, Basic Life Support (BLS) certification
