- Provide patient education teaching sessions.
- Proactively initiate discharge planning with case management to ensure seamless transition home.
- Actively communicate, educate, and plan with referring facilities
- Partner with Regional Sales Manager to execute account business growth plan, discuss opportunities, assess progress, and provide feedback.
- Develop relationships and call on referral sources in person with appropriate hospital protocol.
- Seek out new referral sources on a daily/weekly basis
- Meet/Exceed monthly goals set forth by Regional Manager
- Conduct assessment of patients selected by the hospital for home infusion care.
- Complete and maintain all required reports.
- Help collect any necessary documentation needed in the field
- Licensed (LVN/RN) with at least 2 years of experience.
- Prior experience with customer relations within Sales and Marketing is preferred.
- An action-oriented individual with the ability to: identify opportunity, adapt to change; work independently; and prioritizes tasks/responsibilities.
- Proven success in the completion of assignments.
- Strong customer service skills and service recovery skills.
- Interpersonal skills, including the ability to form constructive and effective relationships with clients and customers.
