    AltaStaff is a staffing agency currently looking for a direct hire Clinic Outreach Representative for one of our Managed Care partners.

    Location: Onsite clinic in Pilsen neighborhood of Chicago

    Pay: $22.00 hourly (Full benefits package to include: Medical, dental, and vision healthcare coverage; 401k with 3% employer match; Flexible Spending Account (FSA); Life insurance; Paid vacation, sick leave, and holidays)

    Schedule: 8:00am - 5:00pm CST

    Position Purpose: This position will work inside a clinic and call patients to complete health risk assessments, assist with scheduling follow-up appointments, and coordinate preventative screenings

    Job Responsibilities:

    • Serve as a liaison between patients and the clinic to ensure outreach and coordination are completed in a timely manner
    • Will perform a variety of administrative, record-keeping, and miscellaneous duties.
    • Call eligible patients to complete health risks assessments
    • Follow up with patients after ER visit to schedule a follow-up appointment with their primary care provider.
    • Contact patients and schedule appointments for preventative screenings
    • Act in a courteous manner and always maintain professional composure
    • Perform clerical duties, such as, maintaining multiple databases, verifying patient information, and updating information as necessary
    • Document all interactions in the scheduling and care management system
    Job Qualifications:
    • High School diploma or equivalent Required
    • Previous administrative, call center, or customer service experience
    • Bilingual English and Spanish preferred
    • Community Health Worker, Certified Nursing Assistant, or Medical Assistant certification preferred but not required
    • Excellent communication skills as well as good judgment and decision-making ability
    • Computer savvy. Must be able to toggle between multiple systems simultaneously
    • Ability to thrive in a fast-paced, entrepreneurial environment
    • Must be able to work onsite within assigned clinic(s)
    • Must be fully vaccinated
