    Millennium Home Care (an Affiliate of Millennium Healthcare, LLC)

    Responsibilities:

    Conduct comprehensive assessments of patients' physical, cognitive, and functional abilities to develop individualized treatment plans.

    Provide hands-on therapy to improve patients' daily living skills, mobility, and independence.

    Implement therapeutic interventions, including therapeutic exercises, adaptive equipment training, and environmental modifications.

    Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure optimal outcomes.

    Educate patients and their families on strategies for managing and adapting to their conditions, maximizing their independence and quality of life.

    Document patient progress, treatment plans, and outcomes accurately and timely.

    Continuously evaluate and modify treatment plans based on patients' evolving needs and goals.

    Stay up to date with the latest research and advancements in occupational therapy practice.

    Requirements:

    Current and valid Occupational Therapist license in the state of Florida.

    Minimum of 1 year of clinical experience as an Occupational Therapist, preferably in a home health setting.

    Strong communication and interpersonal skills to effectively collaborate with patients, families, and the healthcare team.

    Ability to work independently and as part of a multidisciplinary team.

    Excellent critical thinking and problem-solving abilities.

    Proficiency in documenting patient progress and treatment plans using electronic medical records (EMR).

    Valid driver's license and reliable transportation to travel to patients' homes.
