- Provide oversight to all in-home caregivers
- Perform routine in-home supervisory visits per NC State Regulations
- Mentor and guide caregivers within their assigned region
- Provide education to caregivers, staff, clients, and other parties as needed, on clinical issues ranging from disease process to how to appropriately perform hands-on task
- Wound Care
- Medication Box Set up and refill visits
- Catheterizations
- Medication administration
- PIV/PICC dressing changes
- Blood draws
- Community Outreach/Support
- Care Management Services
- Intake Assessments and Consultations
- Performance Management
- Educational Requirement: Registered Nurse
- Ability to interface effectively and professionally with client committees, including family, physicians, attorneys, etc.
- Ability to work in a team environment
- Excellent written and verbal communication skills required.
- PC literacy required.
- Valid driver's license and proof of current auto insurance required.

The RN Care Manager, as part of a care team, will be responsible for the planning and coordination of all services provided to clients and will serve as the clinical expert to all the staff members assigned to their caseload.
