    Job Summary

    This position exists to direct and manage staff, services and outcomes for Transformation Unit of the Transitions, Tribal & Transformations Division. The division is funded in part by the Money Follows the Person (MFP) Federal Demonstration Grant. Minnesota's MFP grant projects are known as "Moving Home Minnesota" and "Moving Home Minnesota Tribal Initiative." The MFP and MFP TI projects are systems transformation grants designed to "rebalance" Minnesota's Medicaid system and services.

    As rebalancing and systems transformation grants, this position is heavily focused on strategic leadership and relationship-building, laying the groundwork for a new collaborative leadership structure across multiple administrations to ensure the department meets the MFP, MFP TI, and state goals to transform our long-term services and supports system. This includes management of the Transformation Team and initiatives, data and research, and team members that support innovation and systems transformation across the department. It will also include coordination of rebalancing initiatives across the department.

    Responsibilities include but may not be limited to:
    • Represent the State of Minnesota and Moving Home Minnesota for CMS, national technical assistance contracts, and various stakeholders in management of the rebalancing mission and activities, to develop and nurture a collaborative structure for the grants.
    • Direct the operations of the rebalancing team and demonstration functions and initiatives.
    • Identify opportunities for the MHM project to enhance Minnesota's long-term care services and supports through rebalancing initiatives. Cultivate collaborative relationships with MHM demonstration partners, key management, and other stakeholders in order to maintain and enhance the demonstration and mitigate project risks.
    • Manage and supervise the MHM rebalancing team. Maintain project staffing and reporting relationships and ensure effective and efficient operations and alignment with strategic and demonstration goals.
    Qualifications

    Minimum Qualifications

    Demonstrated leadership experience. Leadership experience can include:
    • Experience as a supervisor, lead worker, team lead, project lead;
    • Completion of a State of Minnesota leadership program (e.g. ELI, L4);
    • A paid/unpaid position leading a program or chairing a committee
    (e.g. ERG) - Experience must include:
    • 1+ year of experience identifying issues, goals and strategic actions
    • 100+ hours per year time commitment
    AND

    Four (4) years specialized professional experience working with people with disabilities utilizing Home and Community Based Services (HCBS) and programs.

    Experience must also include:
    • Managing projects in a collaborative team environment to produce measureable goals;
    • Knowledge of the following: Medical Assistance, HCBS and programs, federal and state policy directions in long-term care, transition services, housing and employment;
    • Working understanding of state and local service delivery systems for older adults, adults and children with disabilities, and mental/behavioral health
    • Process improvement and program redesign to assess work plans, monitor and evaluate outcomes;
    • General understanding grant management, policy writing, and Medicaid funding rules and regulations;
    • Skills in Microsoft Office Suite including Word, Excel, Access, PowerPoint, Visio, and SharePoint.
    Preferred Qualifications
    • Understanding of tribal nations, rural and urban tribal communities, and the American Indian Worldview;
    • Familiarity with Legislative processes, procedures and protocols;
    • Bachelor's degree public administration, social work, nursing or related field;
    • Presentation skills to explore and solicit discussion from a diverse groups to stakeholders for facilitation of goal oriented outcomes;
    • Experience integrating, coordinating and/or reconciling the interests, mandates, responsibilities, goals and perspectives of a complex and diverse set of stakeholders across agencies and disciplines;
    • Strong human relations and communication skills; communicate clearly and professionally both verbally and in writing; respond openly to requests and concerns;
    • Variety of experiences working effectively with others from different backgrounds and cultures.
