- Collaborates with residents and families to create opportunities for residents to direct their daily care and living experience
- Supports resident's level of independence regarding personal hygiene and personal care needs, including but not limited to dressing and grooming, bathing, dental and mouth care, dining and nutrition, mobility, elimination, psychosocial and safety needs
- Measures, reports and documents vital signs
- Must have weekend availability

With only 39 beds, our Skilled Nursing unit is the perfect size to get to know every resident while providing the best care possible. Clark is a growing organization and provides independent living apartment homes, as well as assisted living, memory care, long-term care, and access to other ancillary services.
