    Happy at Home, LLC is a licensed home health care agency serving the Greater Boston area and beyond.

    We are dedicated to enhancing the lives of all those under our care and providing private care at home, in assisted living communities, rehabilitation facilities, and hospitals.

    Our certified aides are trained to care for dementia and Alzheimer's clients as well as hospice patients.

    We prioritize building and maintaining relationships and open communication between clients and caregivers to deliver personalized care with grace and dignity.

    Role Description

    This is a part-time remote role for a Case Manager.

    The Case Manager will be responsible for providing support and coordination for clients and caregivers, conducting assessments to determine care needs, developing care plans, and monitoring client progress.

    The Case Manager will also facilitate communication and collaboration between clients, caregivers, and medical professionals, and ensure the delivery of quality care.

    Qualifications

    Experience in case management or a related field
    Strong organizational and communication skills
    Knowledge of home health care regulations and best practices
    Ability to conduct assessments and develop care plans
    Excellent problem-solving and decision-making skills
    Empathy and compassion for clients and their families
    Proficiency in using electronic health record systems
    Ability to work independently and remotely
