    Job Title: Quality Improvement Coordinator at Esperanza Health Centers

    Schedule: Full-Time position, Monday through Friday shift with rotating on-call coverage for evenings and weekends.

    Compensation: Competitive salary based on experience and qualifications.

    Wondering about PACE? PACE stands for Program of All-Inclusive Care for the Elderly, a model aimed at providing care for individuals over 55 years old requiring nursing home level care while keeping them at home in the community.


    Primary Duties and Responsibilities:
    • Develop and implement the PACE Quality program.
    • Ensure staff integration into the QI process.
    • Analyze QI activities and oversee PACE Participant satisfaction.
    • Participate in the PACE Participant Advisory Committee.
    • Prepare for federal audits and maintain survey readiness.
    • Stay current with PACE State and Federal regulations.
    • Create necessary reports for quality monitoring.
    • Submit required reports to regulatory bodies before deadlines.
    • Track and trend quality metrics, creating action plans when necessary.
    • Educate the PACE team on quality reporting requirements changes.
    • Collaborate with the PACE Medical Director on improvement projects.

    Requirements:

    • Bachelor's and/or master's degree in health discipline with experience in healthcare.
    • Demonstrated experience in QI activities.
    • Experience in a regulated environment preferred.
    • CPHQ certification preferred.
