- Understanding of healthcare in a post-acute setting and interpersonal relationships.

Have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult situations. Must be able to cope with the mental and emotional stress of the position.

Must function independently and have flexibility, personal integrity, and the ability to work effectively with patients, staff personnel, business and community partners and support agencies.

Job DescriptionEnsemble Health, a division of the Corus Health family of companies, is currently seeking a knowledgeable Transitional Care Coordinator. The TCC coordinates patient transitions through a variety of settings, including hospitals, transitional care facilities, skilled nursing, assisted living, and private residences. Patient-centered transitional care includes communicating with the patients and families to achieve the appropriate care plan, based on patient preferences and clinical statuses. It includes assisting with logistical arrangements, informing and educating patients and loved ones, and coordinating the services of the health professionals involved in the transition as well as in-person assessments in the patient's home.
