Salary Range:

$23.00/hour - $30.00/hour

  • Salary offers will be commensurate with experience and other qualifications._

Schedule:

Monday/Wednesday/Friday: 9:00 AM - 5:00 PM; Tuesday/Thursday: 12:00 PM - 8:00 PM (35 hours/week)

Responsibilities:

The Medication Management Specialist provides various support services to residents with psychiatric disabilities in the Apartment Treatment Program. Reports to the Program Supervisor.

  • Provide oversight and monitoring of client medications at the homes of identified clients who are in need of this service.
  • Link residents with appropriate resources.
  • Engage in ongoing assessment of client needs to determine frequency of medication monitoring.
  • Provide education and resource information regarding medication and medical and mental health disorders.
  • Prepare clients to monitor and take their medications independently.
  • Complete timely submission of medication management progress notes, assessments and other relevant documentation in compliance with department and agency standards.
  • Conduct overall assessment of client welfare and appropriate intervention and followup as needed.
  • Participate in ongoing communication with medication prescribers, service providers, Case managers, and any other collateral contacts as needed.
  • Any other relevant tasks as assigned by management.
  • Provide training on medication management and supervision for Licensed Program staff.
