    Job DescriptionLutheran Social Services of Illinois (LSSI) is a not-for-profit social service agency with more than 100 program sites throughout the state of Illinois. Our services include mental health and substance use treatment; services for returning citizens after incarceration; housing and services for older adults and individuals with ID/DD; and a variety of services for children including foster care and adoption. LSSI is hiring a full-time TransitionCase Manager. Case Manager position is responsible for providing intensive, wrap-around case management services for Williams/Colbert Class Members to facilitate a safe, smooth, and expedient transition from structured facility and into independent living in the community.Benefits and Perks:

    Essential Functions:
    • Provide and coordinate a continuum of treatment services.
    • Carry a caseload of clients providing supportive counseling and concrete services as assigned by Supervisor and operate as part of a treatment team.
    • Ensure all documentation is completed in a timely manner and in accordance with requirements established by Medicaid, DHS, and the Williams/Colbert Consent Decree.
    • Meet productivity and transition targets.
    • Transportation of clients in personal vehicle and agency vehicle is required. Services will be provided in multiple locations.
    • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
    Position Qualifications:
    • High School Diploma with experience in field. Bachelor's degree preferred.
    • 1-3 years of clinical experience preferred.
    • Experience in Crisis Intervention and De-escalation required.
    • Background check clearance required.
    • Demonstrated ability to work independently as well as collaborating with the team.
    • Ability to work in a fast-paced environment, identify concerns and barriers and problem-solving skills.
    • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
    • Excellent organization, presentation and pc/computer skills, including experience using.
    • Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
    • Valid driver's license, in good standing for the state of residency required.
    • Access to reliable transportation required.
    • Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
