As a Case Manager at Vivia Health, you will greatly improve the lives of patients by providing psychosocial support. You will collaborate cross-functionally with an interdisciplinary team to ensure the patient is receiving proper assistance and treatment plans.

What will your role entail?

  • Developing an action plan to improve the patient's overall wellbeing
  • Thoroughly interviewing patients to determine which services and assistance are necessary
  • Serving as a liasion between the client and external organizations
  • Regularly meeting with patients to assess progress and additional needs; modifying plans as circumstances change for the individual
  • Collaborating with a crossfunctional care team to manage patient care plans
  • Manage patient discharge after completion of treatment
  • Advocate for patient to enable them to receive necessary treatment and assistance
  • Maintain the patient's comprehensive clinical record through detailed documentation
This role will be at our Rogers Park location Tuesday to Friday. Schedules are dependent upon the caseload.


What do you bring as a case manager?

  • 35 years of experience with mental health patient populations
  • Bachelor's degree in behavioral health, social services, or human services or a related field

Job Type:
Part-time


Pay:
$37, $45,000.00 per hour


Expected hours:
No less than 20 per week
