    Company Overview: Hamilton Health Center, established in 1969, is the sole Federally Qualified Health Center (FQHC) located in Harrisburg, PA.     What You'll Do:

    • The Medical Case Manager provides specialized support services to persons living with HIV under the Ryan White Part B grant. All medical case management activities are conducted with the goal of optimizing health outcomes.
    • Provide timely and coordinated access to medically appropriate levels of health and support services and continuity of care.
    • Conduct treatment adherence activities with assigned clients to assess readiness for and adherence to HIV treatment(s).
    • Maintain compliance with paperwork and contact requirements for all assigned clients as dictated by program funding organizations and other regulatory bodies.
    • Enter all required documentation electronically in the appropriate database in an efficient and accurate manner.
    • Work collaboratively alongside the department interdisciplinary team to ensure a patient-centered approach to improving health outcomes.
    • Conduct rapid HIV testing in an office and community-based setting.
    • Participate in department outreach activities and patient events.
    • Complete and maintain continuing education credits as required by program regulations.

    Requirements:

    • Case Management Knowledge and Skills: A fundamental understanding of case management functions and elements in the healthcare setting.
    • Tech-Savvy: Proficiency in basic data entry computer skills and the ability to learn new software or databases specific to role.
    • Strong Interpersonal Skills: Ability to communicate courteously and effectively with individuals from diverse backgrounds.
    • Cultural Humility: Awareness of one's own world view and understanding that different world views exist, and cultural knowledge of key populations that will be served to address disparities in services and care.
    • Team Player: An industrious and cooperative nature to foster harmonious working relationships with clinic management, physicians, clinicians, and the public.
    • Bachelor's Degree: A bachelor's degree in social work, psychology, sociology, or other related field is preferred. In lieu of a related degree, candidates with a bachelor's degree in a non-similar field or any combination of relevant experience and education related to the functions of the job will be considered.
    • Valid Driver's License: Ability to travel throughout the service area.

    Preferred Qualifications:

    • Two (2) years of working experience in a case management or comparable social services role.
    • Prior experience working with people living with HIV and conducting rapid HIV testing in a community setting.
    • Knowledge of community resources in Harrisburg City and surrounding areas.
    • Bi-lingual in Spanish
