GHG Home Care is seeking aScheduling Coordinator/Case Manager for our growing office.
We provide high-quality home care to assist the elderly and people with disabilities to live safely and independently in their own homes.
A
Scheduling Coordinator/Case Manager is the person who supervises and directs home health care aides that visit patients who need help in their homes with nursing, light tasks, and/or maintenance duties.
Responsibilities:
- Responsible for scheduling of aides.
- Conducting client assessments and home visits to assess service needs and create and help execute service plans.
- Monitor hours of service delivered, quality of service delivery, client satisfaction and changes in client's functional status.
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction.
- Ability to travel at times to open new cases and visit current consumers.
- Ability to work overtime hours, take after hours calls and be on call for scheduling and Case Management
Requirements:
- Bilingual in both English and Spanish is a PLUS
- Able to be on call for scheduling and Case Management
- Proven working experience in scheduling, case management or other related job field.
- Excellent knowledge of case management principles.
- Effective communication skills
- Excellent organizational and time management skills
- Problem solving skills and ability to multitask
- Basic knowledge of Microsoft Word, Excel, and Google Sheets
- Ability to work in a team environment
- Excellent follow up skills as well as attention to detail
- Must have a Valid Driver's License.
