    Our Center for Endocrinology is seeking an experienced Nurse Practitioner (Endo/Diabetes).


    As a Diabetes Nurse Practitioner working in the Center for Endocrinology at Mercy Medical Center, you will play a vital role in providing comprehensive care to patients with diabetes.

    This outpatient hospital-based clinic serves as a primary care facility for individuals living with diabetes, offering diagnosis, treatment, and ongoing management.

    As a solo provider, you will have the opportunity to work autonomously and collaborate with other providers to deliver high-quality care and improve patients' overall health outcomes.

    Responsibilities


    • Conduct thorough assessments of patients' medical history, physical condition, and symptoms related to diabetes.
    • Perform comprehensive physical examinations, including vital signs, glucose monitoring, and other relevant diagnostic tests, with assistance from support staff.
    • Formulate accurate diagnoses based on clinical findings and collaborate with physicians when necessary.
    • Develop individualized care plans for patients with diabetes, considering their unique needs, preferences, and treatment goals.
    • Prescribe and adjust medications, including insulin therapy, oral antidiabetic agents, and other relevant pharmacological interventions.
    • Educate patients about self-management techniques, including blood glucose monitoring, healthy eating, exercise, and medication administration.
    • Monitor patients' glycemic control, assess treatment effectiveness, and make necessary adjustments to optimize outcomes.
    • Provide counseling on lifestyle modifications, such as weight management, smoking cessation, and stress reduction.
    • Conduct comprehensive diabetes education sessions, both individually and in group settings, to enhance patients' understanding of their condition.
    • Teach patients about the importance of self-care, including monitoring blood glucose levels, recognizing signs of hyperglycemia/hypoglycemia, and managing complications.
    • Offer guidance on meal planning, carbohydrate counting, and healthy food choices tailored to individual patients' needs.
    • Support patients in developing and implementing personalized exercise plans to improve glucose control and overall health.
    • Address patients' concerns, provide emotional support, and empower them to actively participate in their diabetes management.
    • Collaborate closely with the interdisciplinary team, including endocrinologists, dietitians, clinical nurse educators, and other healthcare professionals, to ensure coordinated care.
    • Regularly meet with a collaborating physician to discuss patients' progress, review treatment plans, and explore opportunities for improvement.
    • Coordinate referrals to other specialty services, as needed, to address specific patient needs beyond the scope of the clinic.
    • Maintain clear and timely communication with primary care providers and other healthcare providers involved in patients' care.
    • Accurately document all patient encounters, assessments, interventions, and treatment plans in the electronic medical records system.
    • Ensure compliance with regulatory standards, quality improvement initiatives, and best practices in diabetes care.
    • Contribute to data collection and analysis efforts to evaluate clinical outcomes, identify trends, and implement quality improvement measures.
    • Stay up to date with current research, evidence-based guidelines, and advancements in diabetes management to enhance clinical knowledge and practice.
    Requirements


    • Current, unrestricted Registered Nurse (RN) license and Nurse Practitioner (NP) certification in the state of Maryland.
    • Master's or Doctorate degree in Nursing with a specialization in family nursing.
    • Certification as a Diabetes Educator (CDE) is highly preferred.
    • Prior experience working as a Nurse Practitioner in diabetes care or endocrinology is strongly desired.
    • Solid understanding of diabetes pathophysiology, pharmacotherapy, insulin management, and diabetes self-management education.
    • Proficiency in conducting physical assessments, interpreting laboratory results, and diagnosing and managing diabetes-related conditions.
    • Excellent interpersonal, communication, and patient counseling skills.
    • Strong organizational abilities, attention to detail, and ability to work independently.
    • Proficiency in using electronic medical records systems and other relevant clinical software.
