    Job Title: Registered Nurse Case Manager

    Company: Telescope Health, LLC

    Location: Remote/Hybrid

    Position Type: Full-time

    As a Registered Nurse Case Manager at Telescope Health, LLC, you will be a crucial component in providing quality virtual services to our patients. You will collaborate with internal teams, patients, and their families to ensure comprehensive care coordination and effective management of health conditions. Your expertise in nursing, case management, and telehealth will contribute to improving patient outcomes and satisfaction.

    Responsibilities Include (but not limited to):

    • Conduct assessments and develop individualized care plans for referred patients.
    • Collaborate with physicians, nurse practitioners, social workers, and other healthcare professionals to coordinate patient care.
    • Monitor patient progress, identify any changes in health status, and adjust care plans as needed.
    • Educate patients and their families on treatment options, medication management, and self-care.
    • Document patient interactions accurately and maintain electronic health records.
    • Assist with medication reconciliation and adherence.
    • Coordinate referrals to specialists, home health services, and community resources.
    • Participate in case conferences and team meetings.
    • Ensure compliance with regulatory guidelines and quality standards.
    • Stay informed about telehealth best practices and emerging trends.
    • Use telemedicine platforms and electronic health records (EHR) systems to document patient interactions and update medical records.
    • Troubleshoot basic technical issues to ensure smooth virtual consultations.

    Skills and Qualifications:

    • Active RN license in Florida, multi-state Licensure preferred.
    • Minimum 3 years of clinical nursing experience, case management, and chronic disease management. Telehealth experience a plus.
    • Continuing Education: Ongoing education in telehealth practices, technology, and evolving healthcare guidelines.
    • Critical Thinking: Strong decision-making skills to assess patient needs and respond appropriately.
    • Strong communication skills, both verbal and written.
    • Proficiency in using telehealth platforms, EHR systems, and basic troubleshooting.
    • Ability to work independently and as part of a collaborative team.
    • Compassionate, patient-centered approach to care.
    • Knowledge of chronic disease management and preventive health strategies.
    • Certification in case management (preferred but not required).
    • Telehealth Certification: Optional but beneficial
