Seeking a Registered Nurse to become our Home Health RN Case Manager in the Topeka, KS and surrounding areasAs a Phoenix RN Case Manager, you will be the first point of contact between a patient and hospital.
Our Home Health RNs should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment.
Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.Pay Range:
$65,000 - $90,000 based on 30+ units.
Shift Availability:
Full Time, Monday - Friday: 8am - 5pm. This position carries On Call responsibilities every 8 -12 weeks.
