    Summary:

    Case Manager is responsible for implementing day-to-day and long-term case management services to residents of Guadalupe Home, both in the home and out in the community when applicable. Assess needs of each resident and her child, assisting in the development of comprehensive service plans for the assigned residents. This role also provides case management services to clients in an integrated, multi-cultural setting. The Case Manager will work with community resources and other community-based providers/representatives to coordinate, monitor, evaluate, and advocate for services to meet each resident's specific needs in tracking performance measures and goal path created for each resident.

    Position Responsibilities:

  • *Properly bill TPCN and/or other agency utilized systems provided to residents
  • *Gain a working knowledge of staff policies and guidelines.
  • *Maintain positive culturally competent relationships with residents and staff.
  • *Utilize training in trauma informed consequences when redirecting resident behavior.
  • Documents and reports to House Manager on resident progress, maintain statistical records, and perform other record keeping functions as required by the agency in files and through data entry, including agency software and TPCN.
  • Exercises time management skills to provide efficient and productive case management services to all residents.
  • May attend court hearings, TDFPS staffing's, Circle of supports, etc. for residents on their assigned caseload.
  • Ensure all resident files/records are current with appropriate documentation included, such as admission assessments, service plans/reviews, case notes, goals/reviews, current court orders and appropriate consents and agreements; work/school documentation, educational testing, discipline referrals, etc.
  • Responsible for inputting information; admission and discharge resident data, and measurement data via agency data bases.
  • Assist in development and facilitate implementation of admission assessments and service plans to ensure quality care and compliance.
  • Other duties as assigned by the Program Director, Senior Director or Vice President.

    Requirements

    Minimum Qualifications:

  • Education Bachelor's degree in social work, behavioral science or related field.
  • Experience Minimum of 1-year volunteer or paid experience working as a case manager with vulnerable populations in a child welfare setting or Non-profit setting. Minimum of 3 years' case management experience may be substituted for bachelor's degree.
  • License and Credentials Must be at least 21 years old. Have a valid driver license and clean driving record. Have valid vehicle liability insurance. Ability to pass DFPS, DPS, and Sex Offender background checks.
  • Minimum Knowledge and Skills:

  • Extensive working knowledge of trauma informed care and client advocacy
  • Experience with computer software using Microsoft Suite and Adobe
  • A solid grasp of child development and adolescents
  • Must be detail oriented, organized, self-motivated, work well independently and on a team.
  • Must have good written and verbal skills.
  • Must have good critical thinking and problem-solving skills.
  • Travel Requirements:

    Travel requirements for the position includes __25__% local and __0__% overnight.
