Job Description Job DescriptionThe Care Team Coordinator (CTC) is at the core of Tapestry 360 Healths (Tap 360) team-based care model.
The Care Team Coordinator (CTC) participates on a care team with a provider and one other CTC, working together to meet patient needs.
The role is patient-facing and responsibilities include setting visit agendas, gathering patient information, and performing vital signs.Other tasks include documentation support, facilitating communication between the patient and the rest of the care team, and ensuring patient understanding of their care plan.
The CTC is critical in building trusting, supportive, and healing relationships with patients.The CTC role is an ideal position for someone who wants to work (and learn) in a team environment and is passionate about helping to transform healthcare.
Visit-based skills and experience will be gained through the onboarding process and continuous coaching.Essential Duties & Responsibilities:
Visit-Based Patient CarePre-Visit Planning/PreparationReviews clinic schedule at the beginning of the day with care team to plan for each visitReviews patients medical record before visits to communicate results of medical reports to providerIntakeGreets patient/families in waiting area and rooms themPerforms vital signsWorks with patient to set the visit agendaAdministers point of care testing (fingersticks, urine dips, EKGs)
Performs screening questionnaires such as anxiety/depression screeningsTeam VisitBriefs provider with visit agenda and other pertinent informationProvides documentation support throughout the visitPrepares pending orders (referrals, labs, medications) under supervision of clinical providerEnsures provider is clear in their explanations to patientDischargeReviews care instructions from team visit with patientSchedules follow-up appointments, as indicatedCoordinates care with other care team membersAssists with referral coordinationDiscusses pending labs, administer vaccinations and medications as ordered by providersCare CoordinationActs as a liaison between patients and staff as well as other health organizationsCommunicates with patients about prescription refills, laboratory results, and pending referralsPopulation HealthIn partnership with providers, monitors whether patients are current on preventive care measuresCompletes patient care activities and uses population management tools for patient outreachRelationship BuildingFosters continuity of care for patients by building connections with them across visitsEngages with patients to develop understanding and trustBuilds trusting relationships with other care team membersOperational ResponsibilitiesEnsures general upkeep of the clinical areaCompletes inventory and supplies managementHelps support front desk and participates in daily health center operationsFacilitates huddlesPerforms other job-related duties as assignedImprovement / InnovationContributes to care team meetings by listening to and sharing ideas about process improvementsParticipates in ongoing quality initiativesQualifications:
Education and/or Experience :One of the following is required:Medical Assistant diploma/certificate,orEMT
Experience with data entry, word processing, medical records and/or medical terminology preferred.
Bilingual English/Spanish preferred.
Current American Heart Association or American Red Cross CPR certification required upon hire and throughout employment.
Competencies:
Successfully completes required competency testing annually and as necessary.
Ability to use critical thinking and problem-solving skills to resolve job responsibility issuesAccountability for knowing and actively participating in quality initiatives.
A working knowledge of medical terminology and new developments in the health care delivery system, including federal and state health care regulations.
Familiarity with Centricity EMR preferred.Ability to use software to accurately enter data, and receive, organize, prioritize, and transmit information in a timely manner.
Ability to implement organization policies and procedures and maintain compliance with established risk management and safety procedures, including patient privacy through HIPAA.
