Main Purpose of Job
The main purpose of this position is to carry out case coordination and administrative activities to support individuals and families in accessing appropriate levels of care.
Duties include care coordination for consumers in the Public Behavioral Health System, linkage to services, approval/screening for appropriateness of the requested level of care in the Public Behavioral Health System, crisis management, and homeless prevention.
This position is responsible for monitoring, planning, and administration of community-based programs in the Public Behavioral Health System serving individuals with mental health and/or substance use problems, and their families.
MINIMUM QUALIFICATIONS
Education:
Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience:
Two years of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
