Description

The Psychiatric Rehabilitation Program Case Worker will assist professionals from a wide variety of fields, such as social work, psychology, or rehabilitation, to provide client services, as well as support for families in hopes to improve the social and psychological functioning of children and their families.

• Engages in initial face to face contact with client,
• Conducts a face to face screening assessment of the client within first 5 days to assess individual's rehabilitation service needs and willingness to participate in the mentor services.
• Interviews individuals and family members to compile information on social, educational, criminal, institutional, or drug history and to assess social and emotional capabilities, physical and mental impairments, and financial needs.
• Transport individuals to and from PRP services.
• Designs mentoring services specific to the individuals needs such as peer interaction, effective communication, selfhelp skills, completion of ageappropriate activities of daily living, frustration tolerance, etc.
• Educates individuals or family members regarding behavior modifications, rehabilitation, social adjustments, financial assistance, vocational training, child care or medical care.
• Advocates for parents with child rearing problems and children and youth with difficulties in social adjustments.
• Provides therapist with significant case related updates of clients' status.
• Completes contact notes, documenting relevant contacts with or about the individual.
• Serves as a liaison between student, home, school, therapist, family service agencies, courts, protective services, doctors and clergy members.
• Other duties as assigned.
