    CLASS DESCRIPTION


    The Baltimore City Health Department is seeking a dedicated and dynamic individual for the role of Local Health Improvement Coalition Director.

    This person will report to the Chief Medical Officer and oversee the Local Health Improvement Coalition (LHIC) and various community health projects.

    The LHIC is committed to enhancing the health and well-being of our community through collaborative efforts and evidence-based strategies.

    The LHIC Director will play a pivotal role in guiding the coalition's activities, fostering partnerships, and implementing initiatives to address key health issues.

    The LHIC re-launched in 2021 and is composed of 120+ community, nonprofit, healthcare, and government members.

    The LHIC is at a critical and exciting point. We are executing on the LHIC's current health priorities, which are diabetes, care coordination, and social determinants of health.

    The current efforts of the LHIC include 4 key strategies:
    (1) Educating and enrolling people in Diabetes Prevention Programs (DPP), (2) Developing multi-directional referral systems, (3) Finding solutions for CHW reimbursement, and (4) Supporting Diabetes workforces to address the social determinants of health. The LHIC is also developing the City's Community Health Improvement Plan, which will drive our priorities for the next 3-5 years.


    To ensure the success of these efforts, the LHIC Director will oversee a team of 4 working to regularly convene multi-sector stakeholders, equitably engage community members, facilitate the development of shared goals and priorities, and monitor and track progress.


    The right candidate for this role will be experienced and skilled at centering community, have a strong background in program development and strategic planning, coalition development, and ability to work with and lead interdisciplinary initiatives.

    Experience with social needs/social determinants of health programs, technology solutions, and/or public health outreach will also be an asset.


    ESSENTIAL FUNCTIONS

    • Develop and execute a strategic plan in alignment with the coalition's mission and goals.
    • Provide leadership to inspire and motivate coalition members toward achieving health improvement objectives.
    • Foster a culture of collaboration, inclusivity, and transparency within the coalition and with external stakeholders.
    • Oversee a team of 3-4, including 1-2 managers, 1 data manager, and program coordinator.
    • Cultivate and maintain strong partnerships with local government agencies, healthcare providers, community organizations, businesses, and other key stakeholders in Baltimore City.
    • Oversee team to convene monthly coalition meetings to support collaboration and partnerships.
    • Facilitate regular communication and collaboration among coalition members to leverage resources and expertise effectively.
    • Identify and pursue opportunities for collaboration and resource-sharing to maximize the impact of coalition initiatives.
    • Collaborate with partners and coalition to develop and implement outreach strategies to raise awareness of health priorities in Baltimore City
    • Collaborate with partners and coalition to educate, promote preventive health behaviors, and engage priority populations in Baltimore City
    • Collaborate with partners to organize educational events, workshops, and campaigns to empower individuals and communities to make informed health decisions.
    • Collaborate and support the LHIC and community members to disseminate health-related information and facilitate behavior change.
    • Lead the Community Health Needs Assessment and Community Health Improvement Plan processes every 3-5 years.
    • Oversee the assessment of community health needs and priorities in Baltimore City through data collection, analysis, community-based assessments, and stakeholder input to identify and address our most pressing health priorities.
    • Design and implement evidence-based programs and interventions to address identified health disparities and improve health outcomes in Baltimore City
    • Monitor and evaluate the effectiveness of coalition initiatives, making adjustments as needed to achieve desired outcomes.
    • Support the efforts of the Chief Policy Officer and Director of Legislative Affairs to
    • Support policies and regulations that promote health equity, access to care, and healthy environments.
    • Serve as a liaison between the coalition and Chief Policy Office and Director of Legislative Affairs
    • Oversee and manage a five-year, CDC health equity grant, including compiling data, preparing narrative reports, and ensuring compliance with grant guidelines and deadlines.
    • Develop annual program budgets.
    • Develop and write grant proposals, ensuring alignment with the organization's goals and objectives.
    • Cultivate and maintain relationships with funders and grant-making organizations.
    • Manage contracts and agreements with subgrantees and partners.
    • Develop annual agreements, including budgets and scopes of work in collaboration with vendors.
    • Oversee contractor performance and ensure adherence to project requirements and timelines.

    EDUCATION
    AND EXPERIENCE REQUIREMENTS


    EDUCATION:

    Have a bachelor's or master's degree in public health, health administration, community development, or a related field from an accredited college or university.


    AND

    EXPERIENCE:
    Have at least 3-5 years of experience in community health, coalition-building, or related areas.

    OR

    NOTES(EQUIVALENCIES): Have an equivalent combination of education and experience.

    AND ABILITIES


    • Demonstrated leadership skills, including the ability to inspire and mobilize diverse stakeholders toward a common goal.
    • Strong communication, facilitation, and interpersonal skills, with the ability to build and maintain effective relationships with stakeholders at all levels.
    • Knowledge of public health principles, population health strategies, and health promotion theories.
    • Experience in program planning, implementation, and evaluation, with a focus on addressing health disparities and promoting health equity, particularly within Baltimore City.
    • Experience in grant writing, fund raising, and budget management.
    • Proficiency in data analysis and interpretation, with the ability to use data to inform decision-making and measure program outcomes.
    • Familiarity with Baltimore City's social needs and health disparities, healthcare systems, and community resources.
    • Commitment to diversity, equity, and inclusion principles, with a demonstrated understanding of cultural competency and sensitivity.
    • Ability to work independently, manage multiple priorities, and adapt to changing circumstances in a fast-paced environment.
