    Job Description

    Job Title:
    Chief Health Officer (CHO)

    Synopsis:

    The Chief Health Officer (CHO) is responsible for providing leadership to La Clinicas direct patient care services, including executive oversight of medical, nursing, and mental health and substance use departments, as well as new clinical initiatives in line with La Clinicas strategic directions.

    The CHO is responsible for leading La Clinicas work as a federally qualified health center (FQHC) to improve health outcomes for immigrant Latino families, with a strong emphasis on addressing social determinants of health, linguistic and cultural competency, trauma-informed care, the patient centered medical home (PCMH) model, and behavioral health integration and treatment.

    The CHO is responsible for ensuring that care delivery within all sites meets the highest standards for each clinical discipline and is appropriately aligned with the organizations federally approved scope of practice, and all applicable regulations.

    The CHO is also responsible for cultivating external partnerships to improve patient health and access, and works closely with clinical leaders of sister agencies to advance health equity through clinically integrated networks and various collaborative.

    The Chief Health Officer is an integral part of the Executive Team and works in partnership with other senior leaders to promote a strong organizational mission and culture, develop and drive the organizations strategic work plans, and represent the organization within the community.


    Qualifications:

    Required Education and Experience:
    Graduate of an accredited medical or nursing school (MD, DO, MSN)
    Minimum 5 years experience leading patient care services, with a demonstrated track record of increasing responsibilities
    Demonstrated leadership in areas of clinical oversight, including health information technology, patient safety, clinical risk management, quality and practice transformation
    Proven track record of commitment and work with the underserved

    Preferred Education and Experience:
    Masters degree in public health, health care administration, or business administration in healthcare), strongly preferred
    7-10 years of clinical leadership experience, strongly preferred
    Leadership experience within a Federally Qualified Health Center, strongly preferred
    Knowledge and experience working with Latino, immigrant, and/or uninsured patient populations, strongly preferred
    Knowledge and experience with behavioral health treatment services, strongly preferred
    Knowledge and experience working with clinically integrated systems of care to improve population health, preferred
    Familiarity with health care environment in the District of Columbia and/or Prince Georges County, Maryland, preferred

    Required Skills/Abilities/Certifications/Licenses:
    Licensed and Board Certified to practice in La Clinicas service areas (District of Columbia and Maryland)
    Highly knowledgeable of models, best practices, and trends to advance health equity and improve health outcomes within community-based primary care and behavioral health settings

    Duties and Responsibilities:
    Leadership

    Demonstrate and communicate values that promote high quality, cost effective medical, nursing, and behavioral health services as well as patient and staff satisfaction.

    Serve as a key advisor to the Board of Directors and Executive Director regarding clinical affairs; attends monthly Board meetings and support committees of the Board as assigned.

    Serve as an active member of the Executive Team, working collaboratively to set near and long-term organizational priorities; develop and execute annual organizational budget; promote organizational culture; and drive the organizations strategic plan.

    Works closely with programmatic and community services leadership to integrate programs and services across sites.
    Provide guidance to all direct service departments in their planning and provision of clinical services.

    Represents La Clinica in external coalitions, clinically integrated networks, primary care and behavioral health associations, government and/or funding partnerships as assigned.

    Direct Patient Services Supervision and Management
    Provide leadership, direction, and oversight to La Clinicas direct patient care services and initiatives, including:
    Overall clinical oversight over patient-facing services provided at La Clinicas clinical, community, and school-based settings in the District of Columbia and Prince Georges County
    Set benchmarks and key performance indicators for patient facing services in alignment with strategic plan, including medical, nursing and care management, mental health, outpatient substance use, and school-based mental health services
    Set workplans and training plans with departments to meet key performance indicators
    Approve and ensure accountability for departmental budgets and goals in areas of supervision.

    Ensure that priorities are being set and followed and that adequate resources are available; approve significant shifting priorities, reallocation of budget or staff resources.

    Integrate and coordinate work of the various departments/manage cross-cutting activities.
    Identify and nurture patient service expansion areas in alignment with La Clinicas strategic directions, and identify service gaps.
    Adopt and implement clinical practice guidelines in collaboration with the medical, nursing and mental health and substance use directors.

    Develop, implement and evaluate changes in patient care services based on analysis of clinical and utilization data, changes in community behavior, epidemiology or health problems in the community.

    Identify, foster and nurture external relationships and partnerships to ensure coordination of services for La Clinicas patient population.
    Stay up-to-date in current practice and best practices to advance health equity
    Quality and Clinical Risk Management

    Provide oversight of La Clinicas Quality Management Program for quality assurance and quality improvement as outlined in the Board Quality Management Plan; serve as Chair of the Staff Quality Committee.

    Ensure all aspects of clinical quality assurance are performed according to schedule.

    The program includes an emphasis on continuity of care, medical record documentation, peer review, patient risk minimization, access to care, appropriateness of services and organizational performance.

    Ensure licensed professional staff are in compliance with Federal and local laws and requirements concerning the provision and documentation and billing of medical services.

    Provide oversight to assigned departments to ensure the implementation of quality improvement operating procedures and related assessments, monitor quality improvement outcomes, and update quality improvement operating procedures

    Conduct regular assessments, using data systematically collected from patient records, to ensure adherence to current evidence-based clinical guidelines, standards of care, and standards of practice in the provision of health center services, as applicable.

    Work in close collaboration with Chief of Programs and Community Services and Compliance Officer to develop and communicate quality assurance patient services standards.

    Serves as Clinical Risk Manager for the organization:

    Work in collaboration with Compliance Officer to provide oversight to patient services areas for clinical risk management.
    Work in close collaboration with the Compliance Office for legal activities including medical malpractice, resident and student training and medical record subpoenas
    Provide oversight for day-to-day monitoring of patient safety and clinical risk management activities
    Investigate and reports events, per regulations
    Identify patient safety and adverse events and the implement related follow-up actions, as necessary.
    Communicate analysis and feedback of reported clinical risk management and patient safety information to the organization for action.

    Deliver regular reports and strategies to the Board of Directors which includes: completed risk management activities; status of the health centers performance relative to established risk management goals; and proposed risk management activities that relate and/or respond to identified areas of high organizational risk.



    Supervisory Responsibilities:
    Direct supervision of Medical Director; Nursing, Quality and Population Health Director; and Mental Health and Substance Use Director.
    Recruit, train, supervise, evaluate, and mentor department staff
    Provide timely, appropriate, and ongoing training and development, as well as periodic evaluations and reviews
    Communicate clearly, concisely, and accurately, creating a working environment that provides growth and job satisfaction
    Provide constructive and timely feedback to staff
