    Join Our Team as a Medical Case Manager at Mazzoni Center



    Job Description:
    Assess the needs of individuals living with HIV and help connect them to essential services and resources related to their medical condition and psychosocial needs. Ensure proper monitoring, follow-up, and coordination of services.

    Communication Skills:
    Excellent oral and written communication skills are a must-have for this role. The ideal candidate should be able to communicate effectively with clients, colleagues, and others, contributing to a positive work environment.

    Key Responsibilities:
    • Maintain a caseload of 40-45 clients funded by Ryan White.
    • Deliver a minimum of 400 service units per month.
    • Evaluate the biopsychosocial needs of clients as per program requirements.
    • Ensure regular contact with clients based on care plans.
    • Identify and provide access to necessary resources for clients.
    • Coordinate and oversee ongoing services.
    • Help clients establish external support systems.

    Documentation:
    • Accurately document all client interactions in CareWare and/or athenaNet.
    • Maintain organized client files and uphold confidentiality standards.

    Other Responsibilities:
    • Participate in regular supervision and meetings.
    • Contribute to agency activities and training sessions.
    • Support with food bank deliveries.
    • Perform additional duties as assigned.

    Qualifications and Skills:
    • Must have a Bachelor's Degree in Social Services.
    • Knowledge of HIV/AIDS and community resources is essential.
    • Cultural competency in LGBTQ matters.
    • Experience in working with diverse populations.
    • Proficient in computer skills.
    • Fluency in multiple languages is a plus.

    Physical Requirements:
    • Ability to move around the city regularly.
    • Effective communication skills.
    • Capability to lift items over 35 pounds.
    • Computer usage for up to 6 hours daily.
    • Comfortable working in a moderate noise office setting.
    • Completion of required vaccinations per Mazzoni Center policies.

    Clearances:
    • Background checks including Criminal, Child Abuse, and FBI Clearances are mandatory.
