Sapphire at Gresham is the first Skilled Nursing facility in Oregon to have a ventilation assistance program Our newly renovated, dedicated Respiratory Care Unit provides a person-centered approach to caring for our residents.

All staff receive specialized training approved by the state board.

A Registered Nurse (RN) working in our Vent unit of our Skilled Nursing facility will provide day to day nursing care for ventilator supported residents while working to maximize their quality of life.

This position will assist in the supervision of other healthcare workers including CNA's, CMA's and LPN's.

Ventilator, trach care and vent settings will be managed by RT staff 24 hours a day along with RN PRN support:

Provide primary care to residents in our Vent Unit

Skills such as: wound care, enteral feeding, line care, diabetic management
Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques
Ensure residents' chart is current
Assess resident for change of condition
A Registered Nurse (RN) will assist with new resident admission process
Keep residents' family and physician informed of any changes in resident behavior, medications, or conditions of health
Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc
