    Description
    A community-based non-profit based in NYC is currently seeking Health Educator Coordinators (BSW) for its Bronx location.

    This non- profit provides affordable and supported transitional and permanent housing, specifically designed to assist homeless adults grappling with HIV/AIDS, serious mental health challenges, behavioral disorders, and other chronic medical conditions.

    The Health Educator Coordinator will be tasked with delivering health education and counseling services to individuals affected by HIV/AIDS.

    The base pay range for this position is $30 to $32 per hour.

    The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things.

    Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. This is a true temp-to-perm role; you can gain access to company benefits, paid time off, and more perks once you transition


    Responsibilities:

    • Responsible for conducting health assessments and collaborating with case managers to develop both long and short-term health goals for clients.
    • Provide support in accessing primary medical care, adhering to medication regimens, managing daily activities, accessing nutritional services, engaging in prevention strategies, and accessing other essential services aimed at enhancing independent living skills.
    • Ensure clients are connected to necessary services and maintain awareness of their evolving medical needs.
    • Coordinate health and medical services with the Clinical Team as needed.
    • Inform clients about clinical trial programs in HIV/AIDS programs and provide ongoing health information to clients and staff.

    Qualifications:

    • Bachelors in Social Work
    • 2 years of experience with health education programs and counseling
    • Prior experience working with individuals with HIV/AIDS
