    RN Hospice Case Manager

    Overview Are you a highly skilled and compassionate registered nurse (RN) looking for a rewarding career in hospice as a case manager? If so, we invite you to join our Melbourne Florida team at Amedisys, one of the largest and most trusted home health and hospice companies in the U.Attractive pay $77,000-$82,000 plus quality incentive quarterly payout for excellence in clinical quality and after hours pay.

    *A full benefits package with choice of affordable PPO or HSA medical plans. Up to $1,300 in free healthcare services paid by Amedisys yearly, when enrolled in an Amedisys HSA medical plan.     Responsibilities Performs patient assessments and collaborates with the care team to develop and implement a plan of care. Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Teaches patients, families, and caregivers appropriate patient care techniques. Regularly communicates with clinical manager and care team. Qualifications One year of nursing experience.
