- $1,000 will be awarded in the first paycheck.
- $1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of "Meets Expectations".
- $1,750 will be awarded upon completion of the employee's one (1) year probationary period and a performance evaluation with an overall rating of "Meets Expectations".
- Provide clients with enhanced care coordination with equal focus on both behavioral and primary health in a mental health, *intellectual disability, or substance abuse setting.
- Complete complex assessments, treatment planning, and crisis planning.
- Provide clinical intervention, consultation, supportive counseling, transition planning and other case management duties.
- Will meet with assigned clients within five days of receiving a case management referral.
- Schedule a face to face appointment within five work days with any client coming out of a State or local hospital following an involuntary or voluntary admission.
- Will attempt a collateral or face-to-face contact within five work days after a consumer's release from jail.
- Complete clinical documentation and assures that all filing of clinical information and submission of billing information are done accurately and within the time frame required.
- Complete and final approve progress notes in the Electronic Health Record (EHR) within 24 hours of service being provided.
- May serve specialized populations and/or serve in specialized roles.
- Will provide medically hospitalized clients with support with discharge planning.
- Link with hospital staff and natural supports as appropriate, and will obtain medical records to ensure timely follow-up and quality coordination of care.
- Provide leadership and training to team members on an as needed basis.
