    Description

    Department: Case Management - SRMC

    Position Summary: As a Patient Care Coordinator, you will be responsible for coordinating all systems/services required for a patient-centered care team approach, and ensuring quality patient outcomes are achieved within established time frames and efficient utilization of resources. Your role involves managing patients' treatment course by coordinating care with physicians, nurses and other staff, conducting initial and ongoing assessments, and ensuring continuity of care through discharge planning and utilization of resources. You will also function as the contact person for patients, families, healthcare team members, and community resources as necessary. As a successful candidate, you will require a nationally accredited nursing graduate and have 2 years of directly related experience.

    Qualifications:

    • Program Graduate
    • Bachelor's Degree in Nursing (optional)
    • RN in NM or as allowed by reciprocal agreement by NM
    • CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days

    Physical Conditions: Light Work

    Working conditions: Hazards may include physical risks, dust, fumes, and noise
