    Description

    Nurse Case Manager

    • Provide coordination of patient care services while ensuring quality, cost-effective outcomes.
    • Conduct assessments, coordinate with physicians, and manage patient outcomes.
    • Act as a liaison between patients, families, and healthcare teams.
    • Ensure compliance with hospital policies and procedures and maintain accurate records.

    Qualifications:

    • Graduate of a nationally accredited nursing program.
    • Current RN licensure in the state of employment.
    • CPR certification.
    • Minimum two years of related experience.

    Physical Requirements:

    • Light work, may require extended periods of walking or standing.
    • Occasional exposure to minor hazards.
