JOB PURPOSE
:
The Health and Wellness Program Coordinator is responsible for the execution and oversight of activities and actions within specific programming areas and/or assigned program operations.
In this role the Program Coordinator completes or assists with planning, scheduling, implementation, facility and tech arrangements, and evaluation of program operations; as well as the completion of administrative or programmatic duties, tasks, or assignments that follow well-established procedures and require independent judgment in their execution, within the scope or authority and responsibility for this position.
The Program Coordinator must be able to successfully perform all essential functions, duties, tasks, assignments, and responsibilities for the particular program assigned within the El Rio Health organization at a location assigned by management.
A Health and Wellness Program Coordinator in this position works with occasional supervision and review, and any work problems involving departures from standard policies, interpretations, or procedures are reviewed with the supervisor for direction or resolution.
The primary goal of the El Rio Health - Health and Wellness Program Coordinator is to support El Rio's Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions.
Essential Job Functions:Designs, implements, and delivers evidence-based health and wellness or related programs.
Trains staff community, and organizations in health and wellness topics.
Represents El Rio in the community regarding services and operations available to the community.
Coordinates and Supervises grant programs as assigned.
Tracks and documents wellness program outcome data
Documents biomedical reporting on patient health records
Controls expenses by gathering and submitting budget information, scheduling expenditures, monitoring variances, and implementing corrective actions.
Completes operations by:
Organizing workflow to accomplish established objectives
Developing schedules for the specific program and for the presenters providing the program
Designing, coordinating, and delivering fitness and wellness programs
Delegating responsibility
Implementing productivity and reporting standards
Resolving operational problems
Establishing technical operations
Develops and Implements health and wellness-focused employee, patient, and community health improvement programs and projects
Designs, implements, and delivers lifestyle programs for employees and patients
Coordinates, staffs, and schedules health screening events
Coordinates employee and patient recognition events
Designs and implements highly visible employee, patient and community wellness services and events
Coordinates Patient and Community lifestyle improvement programs:
Manages Health and Wellness program data collection and reporting
Coordinates data needs with IT and Finance
Analyzes and reports outcome data
Implements data needs into program/department strategic planning
Coordinates Health &Wellness medically integrated program data needs with El Rio annual key result goals
Coordinates department annual reporting and awards process.
Controls expenses by gathering and submitting budget information, scheduling expenditures, monitoring variances, and implementing corrective actions.
Establishes IT processes for data collection and reporting
Other Duties and Responsibilities:
In support of the Mission and Vision of El Rio Health, will at all times represent themselves as a professional role model of El Rio Health serving as a positive informational source for members of the community.
Supports El Rio by participating in community events which promote good health, and which contribute to a broader awareness and understanding of El Rio Health and the many services provided to the community.
Participates in all appropriate health care activities related to El Rio Health on a local, state, and national level to maintain and enhance El Rio's image with health agencies, professional associations, and the general public.
Ensures and supports the cost-effective use of materials, supplies, and equipment by limiting waste of all organizational supplies and resources.
Employees must maintain their skills proficiency with El Rio operating systems and/or software programs by adapting to changing requirements and successfully learning and demonstrating new skills in response to software, system, and program improvements or upgrades.
Performs other duties as assigned.Minimum Education and Experience:
Bachelor's Degree in healthcare, health education, public health, social services, education, exercise science, nutrition, health promotion or wellness.
Three (3) years' experience in program coordination, education, community health, or program management in a health-related field.
Three (3) years' experience in curriculum development and outcome data tracking.
Three (3) years' experience with group teaching or presentations in a health, wellness, or education field.
If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.
Required Licenses, Certifications, and Registrations:Certification in a health-related field, fitness or specialized education field
may be required as applicable
to specific wellness program requirements of the position.
Must obtain and maintain current certification in Basic Life Support (BLS) for health care providers.
Employees in this position are required to have reliable transportation that can meet any operational reassignments of the organization during the workday.
If an employee is driving during work hours, the employee is required to possess a valid driver's license and must comply with Arizona vehicle insurance requirements.
Preferred Education, Experience, Skills, Abilities:Masters' Degree in public health, nutrition, exercise science, health education, education, social services, or health-related field.
Five (5)
years' experience in program coordination, education, community health or program management in a health-related field.
Five (5) years' experience in curriculum development, outcome data tracking, and grant management.
Five (5) years' experience with group teaching or presentations in a health, wellness, or education field.
Bilingual (English/Spanish) speaking, reading, and writing.
